Counseling, Advocacy, Referral, Education and Service (CARES) Program

Mission:
Concerned for the welfare of others, CARES social workers are dedicated to assisting patients holistically and providing continuity of care throughout their dental treatment.
Program Objectives:
1. To provide education on and linkage to community resources for new and existing dental patients.

2. To address barriers preventing new and existing patients from receiving oral health care (i.e., transportation, mental health, housing, legal, chronic pain).

How CARES Helps:
The CARES program offers a continuum of care for all School of Dental Medicine patients throughout their dental treatment or one of its affiliates (i.e., Women and Children's Hospital of Buffalo Department of Dental Medicine).  Our qualified staff is available to help you Monday - Friday, 9 a.m. - 5 p.m.

Examples of How We Can Help:
Financial Assistance: 
bulletWe can help you understand your dental insurance, including Medicaid/Medicare applications. 
bulletWe can help you schedule a pre-payment or payment plan for your dental needs. 
bulletWe can help you to address employment concerns.
Transportation:
bulletWe can help link you to community transportation services so you can get to your dental appointments on time.
Health Care:
bulletWe can help you to understand your oral health concerns;
bulletIf you are a caregiver, we can help you with oral health hygiene instruction and/or provide your caregiver with support services;
bulletWe can get referrals to ensure you have good nutrition (food stamp program) and connect you with community health care services;
bulletWe can help you find services if you have a physical or mental disability; and
bulletWe can give you information on long-term care facilities including hospice.
Mental Health:
bulletWe can help you communicate with your dentist, dental hygienist and other dental-related staff;
bulletWe can connect you with community mental health care services;
bulletWe can offer you encouragement and support throughout your dental treatment;
bulletWe can provide cognitive behavioral skills training related to TMD pain or depression
bulletWe can help you overcome dental anxiety
